Objective 
To determine deficiencies in medical patients, accident and preventable deaths in Maharaj Nakorn Chiang Mai hospital. 
Operations. 
1. Set Trauma audit committee.  
2. Build up a record. (Trauma registration form)  
3. To create a report that patients with accidental death. 
4. Create an analysis for factors affecting mortality. 
5. Create a program to record factors that affect mortality from accidents. 
6. Conferencing to examine defects medical patients, accident. 
Step as follows by Secretary of the Board of Directors  1 time/month. 
    1. Collect patient history accident.  
    2. summary of each death reported in a patient who died. 
    3. President scheduled monthly meeting date and time. 
    4. Secretary issuing invitations Committee meeting. Along with copies of accident patients. Who died all delivered before the Conference Committee to study. 
    5. present day meeting about patients report an accident. Who died has to study and follow the steps in the treatment. Cause of death. Factors that affect patient mortality.  
    6. All the deaths that have been approved. Recorded in the program made. 
    7. Annual report. 
Guidelines that patients bring to meetings. 
Accident patients, 160 cases who died all from January 1 - December 31, 2551.  
Result 
Factors affecting mortality of patients. 
The search for more deficiencies in 5 points: 
1.Delay diagnosis. 
2.Error diagnosis. 
3.Error treatment. 
4.Error technique. 
5.System inadequacy.
Conclusion
Deficiencies in medical patients, accident and death are important to prevent the trauma center. So that health provider can follow up effectively care.
