Objective The aim of this study is to describe the prevalence and burden of road traffic injury (RTI) in rural Penglai county, China. 
Methods Analysis was made on the distribution of fatal and non-fatal RTIs, disability adjusted life years (DALY) caused by RTI during 2006-2007. Incidence and mortality information were collected from population survey and death registration system respectively. 
Results Annual mortality due to road traffic injury (RTI) was 34.72/100,000 in total, 58.34/100,000 in male and 13.05/100,000 in female. Maximum mortality rate was for the 30-44 age group. Largest proportion (55.01%) of RTI mortality attribute to motor-bicycle riders. 
The population survey observed 5.59 per 1,000 of incidence rate in total, 7.61 per 1,000 in male and 3.52 per 1,000 in female. RTI incidence was the highest for 30-44 age group. Motor-bicycle riders suffered most (43.82%) of RTIs.
RTI mortality caused 725.17 Years of Life Lost (YLLs) and 55.24 years lived with disability (YLDs) per 100,000 annually. Higher rate of YLLs and YLDs were both observed in male. RTI finally caused 70.59 per 1,000 of DALY rate in total, 104.34 per 1,000 in male, 36.74 per 1,000 in female. 
Conclusions RTI was responsible for large contribution of life year lost. Population aged 30-44, males and motor-bicycle riders should be the focus of RTI prevention.
